Support Coordination (National Disability Insurance Scheme)
Coordination of Supports is to assist to strengthen your ability to design and then build a network of supports that you want and need for you to live your best life!- Connection: assist you to develop knowledge, experience and connections with the community and broader systems of support.
- Support Design: works together with you to understand plan funding and its purpose. Support Coordinator will understand your confidence and skills, and helps you to identify what you want from services. Will develop and design support solutions to meet your outcomes.
- Establish Supports: assist you to identify and consider support options, and link you to the broader systems of supports. Where practical creates a supports and action plan to facilitate your implementation of your own plan.
- Crisis: Assistance to resolve points of crisis and developing capacity and resilience in your network.
- Coach, Refine, Reflect: Coach you through challenges that come up. Helps you to prepare for NDIS Plan reviews.
